Use this form to submit a document to the Organizations Document Library. Valid formats are pdf and image files. Complete the following fields:

Title = name of document
Content (optional) = you can provide a brief summary of what the document contain
Excerpt (optional)
Document Link = click on this dropdown and select File Upload
Category (required) = Find the category where the document best belongs (this is the folder). If a new category is required, email ksergison@verizon.net.
Tags (not used) = (this field is currently not used – leave blank)
